Person Centered Service Delivery is a set of values and strategies used to assist a person in creating a vision for the future and working toward its realization. It is a process that leads to a greater understanding and enhancement of a person’s: Core Values, Gifts and Capacities, Dreams and Visions, Likes/Preferences/Non Negotiable and Opportunities. The course will also provide a variety of Person Centered Planning (PCP) Tools to better support individuals. Some of the PCP Tools covered include:
Planning Alternative Tomorrows with Hope (PATH) – A colorful backwards future’s planning process that enables individuals to plan their future in achievable steps
Essential Lifestyle Plans (ELP) – A collection of tools and ideas that can be used to assist an individual and those closest to them in developing a greater understanding of the person’s values, gifts and dreams
Making Action Plans (MAPS) – A unique tool to help find one’s way on a journey that increases the opportunities for an individual to be included in his or her community
Personal Future’s Plan – An individualized, structured approach to life planning that represents a vision for a more desirable future
***For those who do not have any experience with PATH, Person Centered Service Delivery is a prerequisite to the PATH Facilitator Workshop.
