Connections

Trillium's Connections Program includes Supported Living Services (SLS) and Semi-Independent Living Services (SILS), both of which are provided to adults who live in their own home or apartment. These services are designed for people who may not require 24/7 supervision but simply need some supports to help them maintain independence and lead quality lives in the community. Individuals are mentored in key areas of self-determination and independence. Services may include, but are not limited to, money management, shopping and meal preparation, health and wellness, and establishing community relationships.

Frequently Asked Questions

How do I go about finding my own place to live?
Connections can provide you with initial information about community housing resources in the Duluth area, including affordable housing that is managed by HRA.

Will Trillium help pay my rent?
No. However, most people in the Connections Program live in apartments that are subsidized by HRA, which means you pay only a portion (usually about 30% of your income) of your rent.

What about cable TV and phone?
You are responsible for paying for things like cable TV, telephone, cell phone, internet, newspaper delivery, etc., if you chose to order them.

I don't drive and I don't know the bus routes well. Plus... I haven't done much cooking on my own, and have always gotten help doing my laundry. How am I going to manage all this on my own?
Once you get into the Connections Program, you and your team (usually made up of your case manager, guardian (if you have one), family members, and your Trillium Supervisor) will develop your Personal Outcome Plan based on what your needs and goals are. Menu planning and cooking, budgeting and money management, learning a bus route, and exercise and fitness are all examples of what a Personal Outcome Plan can be based on. Then, when you get together with your staff, you can work on these Personal Outcomes together.
